Il palloncino è rivestito da una miscela 1:1 di paclitaxel (3 µg/mm²) e shellac, una resina naturale. Questo rivestimento amorfo e omogeneo protegge il farmaco durante l’avanzamento. A contatto con il sangue la matrice si idrata, e con il gonfiaggio il paclitaxel passa alla parete del vaso: il rilascio ottimale si ottiene in 120 secondi.

Le sue indicazioni coprono però anche le arterie iliache, infrapoplitee e renali e le fistole artero-venose per dialisi. Indicato per lesioni de novo, restenosi dopo PTA o stent e pre e post-dilatazione di stent periferici. Una terapia che non lascia impianti nel vaso.

Clothoid® della Silvan Medical chiude il sito di puntura femorale con una vera sutura in polipropilene monofilamento 3-0, rilasciata per via percutanea al termine della procedura diagnostica o interventistica. Si usa in arteria femorale comune con introduttori da 5F a 21F e in vena femorale comune fino a 24F. Per accessi oltre 8F si ricorre alla tecnica di pre-chiusura con due dispositivi.
Il posizionamento si conferma senza ambiguità: un lume dedicato porta il reflusso ematico dall’estremità distale fino a un marker esterno, segnalando quando il dispositivo è correttamente in vaso. Gli aghi vengono guidati attorno all’arteriotomia da uno stiletto con piedini, e tutto si attiva meccanicamente in quattro passaggi.
Durante la procedura l’accesso resta disponibile, grazie alla compatibilità con guide fino a 0,038″ e alla valvola emostatica integrata. Il sito può essere ripunto in seguito. Lo spinginodo con taglierino incluso nel sistema permette di portare il nodo sulla parete del vaso e rifinire la sutura senza strumenti aggiuntivi.
